Webbing of the penis is a condition characterized by an abnormally high insertion of the scrotal skin on the ventral surface of the penis. Although it is usually benign, it can cause physical discomfort, psychological distress, or difficulties during sexual intercourse. <\/p>\n\n

1. Understanding penile webbing: Diagnosis and impact<\/h2>\n\n

Penile webbing can be congenital or acquired (following a circumcision or previous surgery).<\/p>\n\n

Clinical diagnosis<\/h3>\n\n

The diagnosis is purely clinical and is made by visual examination and palpation during a consultation. As illustrated in the preoperative photo<\/strong> , when upward tension is applied to the penis, a cutaneous membrane (the webbing) connects the shaft of the penis to the scrotum. <\/p>\n\n

Reasons for consultation<\/h3>\n\n

Patients typically seek consultation for three main reasons:<\/p>\n\n

    \n
  • Aesthetics:<\/strong> When erect, the webbing “conceals” the proximal part of the penis, giving a visual impression of shortening the penis (in other words, a buried penis).<\/li>\n\n\n\n
  • Functional:<\/strong> The stretching of the scrotal skin during erection can cause a pulling sensation, sometimes painful, of the scrotum<\/li>\n\n\n\n
  • Hygiene or condom use:<\/strong> Loss of the normal peno-scrotal angle can sometimes complicate condom application.<\/li>\n<\/ul>\n\n

    2. The Surgical Technique: The “Diamond Shape” Resection<\/h2>\n\n

    The “diamond shape” resection technique is based on the excision of the cutaneous webbing, in a diamond shape, followed by a linear suture.<\/p>\n\n

    The protocol is as follows: <\/p>\n\n

      \n
    • Marking:<\/strong> This is the first and very important step. By applying tension, the webbing is highlighted. We will therefore mark the skin excision area in a diamond shape using a dermographic pen, with the vertical axis corresponding to the height of the webbing. <\/li>\n\n\n\n
    • The Incision:<\/strong> An incision is made along the edges of the diamond shape to remove the identified excess skin.<\/li>\n\n\n\n
    • Dissection:<\/strong> The skin is delicately detached from the underlying subcutaneous tissue (dartos), avoiding important structures such as the corpora cavernosa or the corpus spongiosum.<\/li>\n\n\n\n
    • The Suture:<\/strong> The repair is performed with a vertical suture. The lower scrotal skin is brought closer to the base of the penis, thus recreating the penoscrotal angle and restoring the anatomy. The sutures used are absorbable. <\/li>\n<\/ul>\n\n

      3. Results and Post-Operative Care<\/h2>\n\n

      The post-operative course of this technique is generally simple.<\/p>\n\n

      Home suites:<\/strong><\/p>\n\n

        \n
      • Rest:<\/strong> A few days of professional absence is recommended.<\/li>\n\n\n\n
      • Local care:<\/strong> Daily washing followed by the application of an antiseptic ointment for 10 to 15 days.<\/li>\n\n\n\n
      • Sexual abstinence:<\/strong> A period of 4 to 6 weeks without sexual intercourse (including masturbation)<\/strong> is necessary to ensure solid healing of the area, which may be subject to tension during erection.<\/li>\n<\/ul>\n\n

        The restoration of an anatomical penoscrotal angle is immediately noticeable. The penis appears longer, and the penoscrotal scar is very discreet. <\/p>\n\n

        How can I tell if I have webbed penis?<\/h2>\n\n

        • The double traction maneuver:<\/strong> Facing a mirror (at rest or erect), apply gentle upward traction to the shaft of the penis. Simultaneously, gently pull downwards on the skin at the lower corner of the scrotum. <\/li>\n\n\n\n
        • The visual diagnosis (the frenulum):<\/strong> If you observe a taut skin membrane (like a “boat sail”) connecting the underside of your penis to your scrotum, the normal penoscrotal angle is effaced.<\/li>\n\n\n\n
        • The “short penis” effect:<\/strong> This fusion of tissues masks the base of the penis under the skin of the scrotum, giving the illusion of a penis shorter than it actually is.<\/li>\n<\/ul>\n\n
