Penile curvatures can be congenital (present from adolescence) or acquired (secondary to Peyronie’s disease).

When to consider surgery?

Surgery may be fully justified when the curvature results in:

Congenital curvatures

Origin

It is an anomaly of the development of the penis, without a fibrous plaque.

The deviation appears from the first erections of adolescence.

Symptoms

  • Stable curvature over time (usually ventral).
  • No pain.
  • Sometimes functional discomfort during intercourse, or aesthetic discomfort.

Proposed surgery

The reference technique is the plication of the tunica albuginea (Nesbit technique or variants):

  • Small sutures on the side opposite the curvature to straighten the penis.
  • Reliable intervention, stable results over time.
  • High satisfaction rate (> 90%).
  • Shortening of 1 to 2 cm possible but generally well tolerated.

Acquired curvatures: Peyronie's disease

Origin

Peyronie’s disease is linked to the formation of a fibrous plaque on the tunica albuginea, leading to:

⚠️ a progressive curvature of the penis,

⚠️ sometimes erection pain,

⚠️ decreased rigidity in some cases.

Surgery is only considered when the disease has been stable for several months .

Peyronie's Disease: Surgical Techniques

Surgery is generally offered after the failure of conservative treatment combining PRP injections and penile traction .

Penis plication

Suitable for moderate curvatures (< 60°) and the rods sufficiently long.

A simple technique, effective in 80–90% of cases.

Shortening of 1 to 2 cm possible.

Plaque incision/excision with grafting

Indicated for severe curvatures (> 60°), complex deformations or short rods.

Helps preserve the length.

Satisfactory results in 70–80% of cases.

Risk of postoperative erectile dysfunction in 20–30% of cases.

Penile prosthesis placement

Indicated if Peyronie’s disease is associated with severe erectile dysfunction.

Corrects curvature and restores rigidity.

Very high satisfaction rate (> 90%).
